The practice teacher's demonstration instruction was evaluated using PT form 4. The cooperating teacher and the supervisor serve as evaluators.  And PT form 5 is for evaluating the performance of teachers in practice. This form determines the development of the practicing teacher during the internship experience. The collaborating teacher will assess the final grade.
